When you are ready to submit your payment, check each item carefully before
clicking the "Submit Payment" button below. A verification window from
ProfessionalCharges.com will appear as a popup window.
If the information you submit appears to be complete and error-free,
the verification window displays a summary of the charge you are about to
confirm. You must click the "Submit Charge" button in the popup window to
make final confirmation of the payment. Or if you see a mistake,
close the popup window without clicking "Submit Charge", change the information
and resubmit it.
If the information you submit appears to have missing or possibly incorrect
entries, the popup window tells you what it thinks is wrong and prompts you to
correct the entries it did not accept. You can make your corrections on the
verification page and click the "Submit Payment" button on that page.
(Do not change the License Number shown on that page, LPC004017, because that
number is how your payment finds Anna Kaye Associates.)
When your payment is successfully completed, the popup window will display
a Transaction Confirmed message with a Receipt for Services. You
can print the page for your records if you wish. I will shortly
receive an email stating that you completed the payment.

including any and all positive and/or negative consequences of the service. You acknowledge
